Clinical Observation on Upper Gastrointestinal Bleeding

Abstract
Four hundred and sixty upper gastrointestinal bleeding cases, admitted in Chonnam National University Hospital during the past 6 years from 1975 to 1980 were clinically reviewed.
The results were as follows.
1) The sources of bleeding were esophageal varix rupture (19 %), gastric ulcer (18 %), duodenal ulcer(16 % ), gastritis(12 % ), gastric cancer(10 % ), and unknown orgin(20 %) in order of frequency.
2) Hematemesis combined with melena were noted in 87%, and melena only was in 13% .
3) For the confirmed diagnosis of upper gastrointestinal bleeding, gastroscopy was done in 31% and radiologic study in 49 % of 460 patients. We didn¢¥t find bleeding orgin in 20 % by both diagnostic methods.
4) Among 460 cases, 17% was belonged to massive bleeding group by hematocrit level.
5) Among 460 cases, 317 cases were treated conservatively and 143 cases received operation. The over-all mortality rate was 24 % (109 cases) and the mortality rate for patients treated convservatively was 29 % (93 cases), while for those surgically treated it was 11%(16 cases).
6) A group of gastrointestinal bleeders receiving 1~4 pints of whole blood transfusion was 38 % and 16 % of patients did not receive any blood transfusion.
7) The annual incidence of upper gastrointestinal bleeding from 1975 to 1980 revealed rather increasing tendency, but decreasing tendency was observe in mortality rate.
